    *Complete each section of this fillable registration form.

    *After completing the Church Contact and Senior Pastor Contact sections, you will be able to save your partially completed form. Click on the save button at the bottom of the form. To finish, follow the instructions that will be sent to the email provided.

    *After you complete all sections of the form and click on the “submit” button, you will be taken to a screen to make your church’s first Floodgates Collective payment (monthly, quarterly, or in its entirety). Your payment finalizes your church’s registration. For this reason, please be prepared with your church’s credit card or bank account information when you click to submit the completed form.

    *You will receive an electronic confirmation of your registration form and payment for your records.

    *You will also receive an electronic copy of your completed registration form as you submitted it.

    • YES, we understand that upon acceptance into the Floodgate Collective, we will receive instructions about how to identify, select and assemble an 8-person team of laity who are committed to attending the Floodgates Collective trainings with the senior pastor, to participate in the coaching sessions, and to partner with the senior pastor and others to bring implementation across the entire congregation.
